Southeast Health provides patients with the opportunity to take an active role in their own healthcare while keeping parents or legal guardians informed by providing access to the patient’s portal via proxy access.

Patients or their parents/guardians must submit the appropriate proxy form to either the Southeast Health Medical Records Office or their physician’s office or signup online for MyChart accounts to be established and proxy access granted.
