Preparing for your appointment
To ensure all patients receive a thorough evaluation and the best care in a timely manner, we kindly request that you bring the following on the day of your appointment:
- Insurance ID cards
- Identification card, driver’s license or government ID
- Co-payment or pre-payment
- X-rays, MRI images, and other diagnostic imaging films or discs
- Medication list (please be sure to include the dosage and instructions)
We kindly ask that new patients arrive 30 minutes prior to your appointment time and that established patients arrive 15 minutes prior.
