Here is what to expect:

• If you have been referred by your primary care doctor or another specialist, the referring physician will make your appointment and share your medical records with us. It is vital that we receive your prior to your visit in order for the surgeons to treat you appropriately. Our staff works diligently with your referring provider to ensure that this information is received in a timely fashion so that you have no delays at the time of your visit.
• If you are making your own appointment, we may ask you to complete a medical records release form to obtain your records from your physician.
• If you have recently had any diagnostic imaging tests (Ultrasound, CT scan, etc.) related to your condition, please share this information when you request an appointment with our office. If your test was completed at Southeast Health or Flowers Hospital, we can obtain your images. If tests were completed at another facility, please bring an electronic version of your images to the appointment.
